Rehabilitation Services

Physical Therapy

Physical therapy helps patients restore strength, balance, and mobility following injury, surgery, or illness. Through personalized treatment plans and guided exercises, therapists work to improve movement and help patients safely return to daily activities.

Occupational Therapy

Occupational therapy focuses on helping patients regain independence in everyday activities such as dressing, bathing, eating, and household tasks. Therapists guide patients in improving coordination, strength, and practical skills needed for daily living.

Speech Therapy

Speech therapy supports patients who experience challenges with communication, cognition, or swallowing. Therapists help improve speech clarity, memory, problem-solving skills, and safe swallowing for better overall quality of life.

Smoke has been a part of the Reliant family since 2019, residing in several of our facilities over the years. After his most recent transition from Senath South to Four Seasons, a new chapter began. With teamwork, dedication, and a shared goal, we started the process of helping Smoke move into an Individualized Supported Living (ISL) home within the community. On August 1, 2025, that dream became a reality. Smoke’s commitment to the Positive Pathways Program and his determination to better himself are clear reflections of his resilience and growth.
